Position Summary
An Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing.
In accordance with physician orders and plan of care the Occupational Therapist (OT) carries out therapy services designed to restore functions, diminish or correct pathology, promote and maintain health, and facilitate learning of those skill necessary to productive living of assigned patients.
Essential Duties and Responsibilities
- Conduct initial evaluations and develop treatment plans.
- Administer prescribed therapy and skilled procedures.
- Collaborate with physicians and healthcare team on care planning.
- Educate patients and families on home programs and exercises.
- Maintain accurate clinical documentation (evaluations, progress notes, discharge summaries).
- Participate in agency in-service training and quality reviews.
- Supervise therapy assistants and aides as needed.
- Ensure compliance with healthcare regulations and standards.
- Communicate effectively with patients, families, and medical professionals.
- Use resources efficiently and maintain professional conduct.
- Support agency goals and participate in continuous improvement.

- Strong communication, judgment, and supervisory abilities.
- Ability to perform physical tasks and work in high-pressure environments.
- Reports to Director of Nursing; may supervise assistants and aides.
- Professional appearance and hygiene required.
- May include stethoscope, blood pressure cuffs, and therapy tools.
Education & Experience
- Preferably 2 years in home/community occupational therapy.
- Graduate of an accredited OT program; certified and licensed in Texas.
