Position Summary
As a Home Health Care Community Liaison you will be able to establish and maintain an effective working relationship and open channels of communication between agency, medical community and the public served by the agency. A results-oriented marketer with strong interpersonal skills and a knack for building relationships.
Essential Duties and Responsabilities
- Maintains communication with hospital, social services/discharge planning, physicians and other appropriate personnel to establish effective working relationship between the public served by the agency and the agency.
- Serves as a link to physicians and hospitals to identify needs and maintain effective working relationships.
- Stays abreast of trends, which may effect agency operations such as Telehealth and in home testing such as X-rays, EKG, or Phlebotomy.
- Assists the Health Care Liaison Manager with the development of better agency connections with physicians and other health related providers in the community through personal contact.
- Provides ongoing education/in-service to the agency, medical community and the general public.
- Represents the agency at pubic outings and community activities as needed. Oversees preparation of informational resources publications, health fairs and public speaking.
- Assist the Billing Manager by facilitating the retrieval of Doctor's orders and 485's.
- Monitors doctor and patients expectations and satisfaction.
- Facilitates the flow of information, problem solving and educational needs between the agency and assigned clients.
- Networks with physicians and other medical professionals.
- Attends weekly/monthly staff meetings.
- Submits reports weekly.
- Demonstrates working knowledge of agency policies and procedures and Medicare Part A and Part B.
- Adheres to agency policies and procedures.
- Develops and maintains accurate promotional materials to connect with public served by the agency.
- Maintains confidentiality of client information.
- Maintains security of computers, data and hard copy files at all times.
